Directions and Informed Consent
Title: An investigation of the role of counselors in the return to work of military reservists with disabilities
Investigators: Dr. Michael Frain, and Dr. Malachy Bishop
Purpose: The purpose of this study is to examine how employment and employment services change for individuals who acquire a disability during military action.
Procedure: You will be asked a series of questions concerning work and your experiences with deployment. Once you are finished with the questions your information will be put into a computer database with no information that will make you identifiable. The survey should take approximately 30 minutes to complete.
Risks: There does not appear to be any significant risks to you in answering these questions.
Benefits: The benefits of this study may include information concerning how to best serve you return to work needs. You will also receive a $20 gift card of your choice after completing this survey. You may also donate this gift card to Wounded Warriors if you wish.
Data Collection and storage: All of the results will be kept confidential and secure and only the people working with the study will see your data, unless required by law.
